Our RNs will visit you in the hospital and our caregivers will drive you home. We’ll lend a hand with errands such as grocery shopping, medication pick-up, and transportation to appointments - and assist you with post-op exercises and treatments, etc.

  1. Share Our Information — Share the Home Helpers Home Care brochure and the Free Guide to Choosing In Home Care with the client.
  2. Client Consult — Home Helpers will provide a no-cost consultation for the potential client with our Care Manager to begin preparation for return home.
  3. Discharge Care Conference — We will work collaboratively with the client, client’s family and hospital care team to plan for all post discharge care needs.
  4. Home Visit — We will conduct an evaluation of the home to ensure it is safe and ready for the client’s return.
  5. Discharge Day — We will assist with discharge plans, such as transportation, if needed. Once home, we will review the care plan with the client, introduce the caregiver(s) and then start our in-home care services.

Steps to Care

    Initial Call Once we’ve received your information, we will have a detailed call to gather more information on the care situation, answer any pressing concerns, and set up a time to meet in person.
  2. 2.
    In-home Care Assessment Here we will answer all your questions, and ask some of our own to craft a Cared-4℠ Customized Care Plan to your unique needs.
  3. 3.
    Start of Care We will come with our Caregiver on the first day of care to go through the Care Plan together and help with introductions.
  4. 4.
    Client Visits Once we have begun care, we will pop in at times our Caregiver is there to see how things are going and determine if we need to adjust the Care Plan.
